Prosthetic Rehabilitation of a Patient with Nose Defect Case Report Authors: Satyabodh S Guttal, MDS,MFPT 1, Blessy Bangera, BDS,2 Adarsh Kudva,  MDS,3 Basavaraj R Patil, MS, 4 Abstract Midfacial defects are enormous defects that result from cancer treatment that rarely are corrected by surgical reconstruction alone; they generally require a facial prosthesis to restore function and appearance. Surgical reconstruction may be viable for few defects, which are done with different flaps. But for the total nasal resection, prosthetic option would be more feasible. Nasal cartilaginous anatomy is complex due to the varying contours. Therefore it may be difficult for the surgeon to reconstruct the entire nose. This clinical report describes the rehabilitation of a large midfacial nose defect with a dental implant retained nasal prosthesis. The patient had adenocystic carcinoma of the medial maxillary wall extending to the nose. Introduction The face being the most noticeable part of the body when disfigured may lead to an impaired social life stemming from esthetic problems. 1, 2 Among facial defects, nasal defects produce severe cosmetic impairment. . Rehabilitation of such defects subsequent to surgery is done in a sequential manner, which includes a surgical, provisional, and definitive prosthesis. 3 Prosthesis helps restore the patients self-esteem and confidence, hence affecting the patients and their life style. 4-6 Adenoid cystic carcinoma (ACC) is a rare malignant perineural tumour of the major and minor salivary glands, accounting for 2% of all head and neck malignancies and approximately 10% of all salivary gland neoplasms. 60% to 70% of ACC’s arise in the minor salivary glands, which may be localized in the palate, paranasal sinuses and nose, although they may also occur in the parotid or submandibular glands.7 In the past, nasal prostheses were held in position with strings or straps fastened behind the head,8 intranasal or intraoral extensions,9,10 and gold strings or leaves.11-13 Spectacle frames have been accepted for securing nasal prostheses.14,15 Today, with the development of biomaterials, prosthetic substitutes are secured with readily available adhesives that are effortlessly applied 16 however, the effectiveness of adhesives is questionable considering presence of mobile tissues in the defect, nasal secretions, and moist air associated with respiration.17 These factors would compromise the adhesiveness. The concept of osseointegration 18 has enabled a more reliable mode of retaining nasal prostheses. 19 This clinical report describes the rehabilitation of a large midfacial defect using an implant retained nasal prosthesis. Clinical Report: A 63-year-old female patient who reported to the B.R Patil Cancer hospital, Navanagar, Dharwad was diagnosed with adenocystic carcinoma of the medial maxillary wall. Patient had no medical co-morbidity. Patient gave history of nasal obstruction due to nasal mass on left side of the nostril for which medial maxillectomy was done via endoscopic approach in the year 1993. Then in 2012 she reported back with the complaint of nodular swelling over nasal dorsum with tearing and nasal obstruction with no orbital symptoms. Intra-orally patient had destruction of palate on the left side crossing midline. Upon further investigation, biopsy revealed adenocystic carcinoma of the nose and left maxilla with no involvement of orbit or anterior skull base (Fig 1). Two cycles of chemotherapy with cisplatin, 5 flurouracil and paclitaxel according to body surface area was given. The defect resulting after excision had to be covered at the earliest. Hence, prior to surgical intervention, prosthetic consultation was suggested to the patient who was thus referred to our Department of Maxillofacial Prosthodontics. Since an immediate definitive prosthesis was not feasible, the patient was suggested for temporary rehabilitation with an interim silicone nasal prosthesis with an attached eyeglass frame. However, since the patient expressed her displeasure towards spectacles for lifelong usage, she was given the option of implant-retained definitive silicone nose prosthesis. The patient agreed for the same. An orthopantomograph and computerized tomography scan were made as a part of the investigation to evaluate the bone height for implant placement. Left total maxillectomy with palatal resection across midline and total nasal resection done via weber ferguson incision, left modified radical neck dissection type three via macfee incision was made. The glabellar bone was evaluated on the operation table and upon conclusion that adequate bone was available; a single implant of 4.2 diameter x 6.5mm length, (Toureg S; Adin implants, Nazareth, Israel) was placed (Fig 2). The advantage of placing the implant on the operation table was that the patient would be under general anesthesia, and the psychological trauma of undergoing another surgical procedure was avoided. Following a healing period of 3 months the open tray impression posts were placed and the final impression was made. The abutment was placed on the implant and a custom made acrylic sleeve was fabricated for the abutment (Fig 3). A wax sculpted nose on the master cast was made to adapt to the margins of the healing wound. On either sides of the acrylic resin sleeve, two neodymium-iron-boron magnets, 5mm diameter x 1.2mm thick (Magnatech; Mumbai, India) were embedded into extensions made out of autopolymerising resin. The structure hence resembled a winged sleeve which was cemented on to the abutment using zinc-phosphate cement (Harvard Dental, Hoppegarten,Germany) (Fig 4). An acrylic resin index was fabricated over this structure which would harbor the respective magnetic keepers. The acrylic index was placed at its position over the magnets and was picked up by the wax nose that was placed on it using a drop of cyanoacrylate. The resulting wax nose thus incorporated an acrylic index with the magnetic keepers. This wax nose was carefully invested and the packing procedure using silicone and acrylic resin border framework, intrinsic coloring was carried out as mentioned for the interim above. Extrinsic coloring and pigmentation was done and patient was happy with the esthetic results. Digital weighing scale revealed that the definitive nasal prosthesis weighed around 12.2gms. The retentive force offered by the two neodymium-iron-boron magnets (Magnatech; Mumbai, India) was found to be 7.2N. The prosthesis was delivered to the patient (Fig 56). Following this, home-care instructions were given. In the subjective evaluation, the patient was very happy with the esthetics outcome of the prosthesis and expressed her great pleasure towards her ability to swallow liquids. The ryles tube continued to remain in place considering the general health condition of the patient and the need to feed semi solid food and protein supplements. The prosthesis was light in weight and could be comfortably placed in position as it was self-aligning due to the use of magnets. Patient, who is now on regular periodic follow-up ie, recalling at every 3 month period, is found to be doing well. Discussion Nasal reconstruction modalities comprises of primary closure, healing by secondary intention, skin grafts and local flaps and regional flaps. Small surgical defects can be treated well with different types of local flaps. The forehead flap is the better option for the large nasal defects. 20 The complex anatomical configuration may cause difficulty in surgical rehabilitation. In such cases, prosthetic closure is predictable and hence usually the treatment of choice. 21,22 The breakthrough for rehabilitation of facial defects with implant-retained prostheses came with the development of the modern silicones and bone anchorage. The limitations of the prosthesis were explained to the patient prior to the treatment, that fact that the prosthesis would enhance esthetics but would contribute less to the functions like speech and masticatory habits. Hence, the patient had no psychological set back on the prognosis of the treatment. In addition, there was a major set-back in terms of achieving outstanding esthetical and functional outcome due to the fact that all the work was carried out under technical constraints. This included a lack of time, chair-side patient availability, and ideal light conditions which, to an extent precluded optimal color blending. The main objective of treating this case was to close the open defect, to prevent the further spread of infection in the soft tissues exposed to the environment. The use of a magnetic assembly has eliminated the need for use of spectacle retention as per the patient’s request. The patient indicated that the nasal prosthesis reduced self-consciousness and was comfortable to wear without any type of irritation to the surrounding skin. Cultural Diversity in the Workplace | Literature Review In todays increasingly globalized world, the phrase diversity in the workplace tends to pop up everywhere. The main reason being that modern technological advances in transportation have shrunk the globe to such an extent that it is no longer a problem to move to any part of the world be it for entertainment, studies and especially work. This paper explicitly focuses on literature that relates to cultural diversity in an organization and its implications on employee performance and the performance of a corporation as a whole. It will also take a look at cultural diversity from either side of the mirror, that is, the positive as well as the negative aspects and how to manage such an employee base. Please also note that, while there has been some debate on whether race and culture should be thought as the same thing, this paper regards them as inter-usable so as to avoid confusion. So what exactly is cultural diversity? It has been described as the composition of work units (work group, organization, occupation, establishment or firm) in terms of the cultural or demographic characteristics that are salient and symbolically meaningful in the relationships among group members (DiTomaso, Post, Parks-Yancy, 2007) .The time has gone by when you expect an employee from a different cultural background to just blend in and start producing results (Amaram, 2007). Organizations have realized that a multi-cultural workforce can prove to be a great asset especially if the business is to be expanded to international markets. So the focus has somewhat shifted to come up with strategies to handle such an employee base and how to sustain that program. In his assessment of the value of a multi-cultural workforce, (Konrad, 2003) states three reasons why companies are changing their attitudes and strategies regarding recruitment of employees with different backgrounds. Firstly, the best talent is not always local; you need to look globally to recruit the best minds in the business. Secondly, he talks about market share, that is, a culturally diverse workforce can better cater to an increasingly diverse customer base, hence increased market share. The insight that a local can provide into the workings of the local market and also of the mindset of the local consumer is invaluable. Finally, he states that each culturally different individual brings with him something different to the table, and when you combine all these different attributes, it results in making the organization more competitive. In a non-business related argument, (Jayne Dipboye, 2004) suggest that increasing workforce diversity is the ethically right thing to do an d also that it is a better way to address race and gender issues. This argument may seem insignificant considering that the current competitive environment means companies care more about capital than the right thing to do. But (Amaram, 2007) brings up a very interesting argument about why corporations are moving to diversity programs. In addition to ethical and social responsibilities, he says that these programs have become necessary to avoid legal problems namely due to civil rights laws. Furthermore he states that to gain competitive advantage corporations have been forced to bring forward new strategies to bring out the best from a diverse workgroup and avoid conflicts. Even at the Govt. level it has been realized that effectively managing cultural diversity is vital to economic success. One example is the Australia Govt. policy, where diversity management is governed by a state policy namely, Productive Diversity. The aim of this policy is to make full use of the experiences and skills of people raised overseas, therefore gaining priceless Intel on international markets. (Leveson, Joiner, Bakalis, 2009) However what has been discussed above is based on theories and rhetoric. Is there any solid evidence that cultural diversity leads to a prosperous organization? The answer is a big maybe. Countless research and studies have looked at this issue from many different angles, each with their own differing conclusions. After reviewing various researches on this issue Jayne Dipboye (2004) came up with four conclusions regarding the relationship between cultural diversity and organizational performance. Increased diversity does not necessarily improve the talent pool Increased diversity does not necessarily build commitment, improve motivation, and reduce conflict Increased group-level diversity does not necessarily lead to higher group performance. Diversity does not necessarily improve organizational performance A study (Choi Rainey, 2009) suggests that it all comes down to the type of diversity i-e race, gender, religion etc. The findings were consistent with previous research (Milliken and Martins, 1996; Williams and OReilly, 1998) that suggest that racial diversity is inversely proportional to organizational performance due to less cohesion within the group and more conflict. It should be noted however that this study was specifically based on US federal agencies so it cannot be considered to represent the global business environment trend or even the national business environment of the US as a whole. Various other studies on the subject matter provide some very interesting results. McMahon (2010) in her article surveys the more recent studies on diversity. Here I would like to talk about two of them. First, (Richard, 2000) carried out a research on the US banking industry. He chose 63 banks from 3 states based on 3 different factors: California (high racial diversity), Kentucky (low racial diversity) and North Carolina (financial wealth). The study showed that there was no direct relationship between diversity and performance instead it depended more on the firms business strategies. When the strategy was more focused on growth there was a positive relation between diversity and performance but the result was opposite when the strategy shifted towards downsizing. Again, the above study, even though regarded as valuable information, only included the US banking industry so it cannot be said to depict the complete picture, (Richard, Murthi, Ismail, 2007) therefore widened the scope to other industries and studied the relationship between racial diversity and intermediate as well as long term firm performance and the role that environmental context plays in this. The results showed that at high as well as low levels of racial diversity, there was a curvilinear positive relation between racial diversity and intermediate firm performance. The result also showed a positive correlation between racial diversity and long term firm performance. But these results were dependent on the type of industry and environmental stability, meaning that the U-shaped relationship between racial diversity and firm performance would be weaker in unstable environments compared to a stable one. Till now I have reviewed how cultural diversity affects businesses and also related the theoretical assumptions to actual studies done on this very important issue. In the next portion I will try to review literature that primarily focuses on the best practices for managing diversity. Effective management of diversity is based on recognition of commonalities and awareness of differences (McMahon, 2010). Managing cultural diversity involves identifying the best practices. A best practice is a technique, method, process, activity, incentive, or reward that is believed to be more effective at delivering a particular outcome than any other technique, method, process, etc. when applied to a particular condition or circumstance (Wikipedia, 2010). So what are these best practices that I referred to? Amaram (2007) gives a few suggestions about them. He gives particular importance to diversity training programs, giving managers diversity related goals and making sure they are met and finally, giving respect to people of all races, that is, making room for religious holidays, diet preferences etc., because sometimes its the little things that matter the most. Jayne Dipboye (2004) suggest conducting a thorough needs assessment so that the diversity initiatives are tailored to the situation. Another suggestion is to develop a strategy keeping in mind the environmental context of the organization thereby echoing the results of the research carried out by Richard, Murthi and Ismail (2007). One more point that they touched in their paper was the need for inventing a system to evaluate the effects of diversity management strategies developed by organizations so that new and improved strategies can evolve by learning the mistakes from previous ones. CONCLUSION The increasingly diverse workplace is consistently posing newer challenges for an organization. Therefore there is a need for organizations to realize the importance of devising diversity management strategies. These strategies require thorough planning and looking at all aspects as well as the resulting consequences that will arise from a particular strategy because a good strategy can do wonders for an organization. At the same time these strategies could backfire if there is a lack of planning or research before implementation. After reviewing the above mentioned literature, one can see that cultural diversity is a complex phenomenon. Its role in improving organizational performance depends on various factors, for example, the environmental context, the type of industry, organizational strategies, etc. Furthermore cultural diversity alone does not guarantee immediate, tangible improvements in organizational, group, or individual performance. Nevertheless, achieving a culturally di verse workforce and effectively managing this workforce can yield huge benefits. Patricia J. Williams While most pundits of America's social and political discourse are either beating dead horses or tilting at windmills, Patricia J. Williams seeks out the racist, sexist, heterosexist, and classist forces that underlie a number of socio-political pathologies. Williams' regular Nation magazine column, "Diary of a Mad Law Professor" is curious in that it often evokes visceral negativity in casual readers. It certainly affected me that way. At first it was difficult to get beyond the name of her page; thumbing through each issue I sheepishly wondered what this crazy lady would get bent out of shape about this week. Though I generally agreed with her ideas, it struck me that Williams was too radical (as if there really is such a thing in a mainstream media culture that chooses to wear blinders). Williams vigorously uproots conventional wisdom as she strips away the "rich-white-male"-centric viewpoint; power and a voice are given to those who simply are acted upon. Like Howard Zinn who has promoted a view of history through a populist lens, Patricia Williams promotes a viewpoint that examines and judges the treatment of the marginalized. Williams is clearly not the only contemporary essayist with a broad-minded viewpoint on social issues. Katha Pollitt, E.L. Doctorow, Gore Vidal, and Alexander Cockburn, among many others, are similarly progressive in their opinions on society, politics, and culture. Williams, however, has a modified gestalt upon which her liberal commentary about socio-political affairs is based. The way in which the mechanics of society can be explained is a relationship of dominance and submission, a pornographic association. The end of the Soviet Union will be remembered also because it had such a great impact on all its republics. The Soviet Union was not a horrible place though as it helped all the poor countries stay strong since they were united as one union. Instead of being a whole bunch of third world countries, their unity in communism did work at keeping the people safe. For example Yugoslavia was around as on nation back then because of communism but once the collapse of communism Yugoslavia broke apart. Why many people feared the USSR, the Union of Soviet Socialists Republic, or CCCP, USSR spelled in Russian, was because the fact that a lot of missiles would frequently disappear from the missile silos that belonged to the Soviet Union, and because of this it shows that the Soviet Union wasn't to safe of a country to live in and that the people there weren't easily trusted so other countries near the borders of the Union could be in great danger. Topic Literature Review: English Language Teaching Strategies for Learning-Disabled Secondary School Students Introduction   Ã‚  Ã‚  Ã‚  Ã‚  One of the aims of the Singapore Ministry of Education is to ensure that all school-going children receive a minimum ten years of general education. Streaming is one way to ensure that all students are taught according to their academic ability, and â€Å"learn at a pace which they can cope.† (Coping with Singaporeans’ Concerns, 2001, p. 4). At the primary school level, remediation programmes such as the â€Å"Learning Support Programme (LSP) and the Encouragement Achievement and Better Learning (ENABLE) Programmes† are made available to assist students who have been assessed as weak in English and Mathematics (Coping with Singaporeans’ concerns, 2001, p. 4). At secondary level, the Normal Technical (NT) stream was implemented in 1994 to cater to students who are less academically inclined. The majority of the learning-disabled students will be found in the NT stream. In NT stream, students follow a less rigorous curriculum which focuses on English and Mathematics, and more hands-on learning experiences. The NT curriculum basically prepares students for vocational/technical academic and career paths. Unlike primary schools where there are remediation programmes to support students who are identified as weak in academic performance or â€Å"learning-disabled†, little attention is given to NT stream students, especially students with learning disabilities. This can be attributed to a number of reasons such as exclusion of NT stream students’ performance at National Examinations from the school league table, and inadequately-trained general education teachers to teach students with special educational needs. For any remediation programmes that are available, they are provided at an ad hoc basis by out-of-school â€Å"ethnic self-help groups and voluntary welfare organizations† (Coping with Singaporeans’ concerns, 2001, p. 4). Research has revealed that NT students suffer from â€Å"low self-esteem†, â€Å"achievement motivation†, and â€Å"poor study habits† as they progress up the levels in secondary schools (Chan, 1996). One of the main reasons for the poor academic performance of NT students is that most NT students face problems following lessons in class. When consultants and client clash: Problem Essay Statlers have failed to get their clients to acknowledge the differences in thinking about the merger. The rationale behind every merger is that the sum is greater than the parts. Typically, clients identify synergies for the merger and from then on consultants suggest the decisions necessary for attaining them. The synergy cited in this case, economies of scale, is only possible if the two firms worked together as a single unit.Susan Barlow ¶s lack of experience in conducting with clients and failure to understand the need for merger coupled with Kellogg ¶s ineptness in handling sticky situations has led to the current state. Susan, in her initial briefing with Mr. Kellogg, started off on a wrong note. First, she patronized the entrepreneur-turned-CEO, accepted his list of interviewees and even agreed to his deadlines. If she was any experienced, she would have been more pro-active, played the role of a devil ¶s advocate to expl ore other views about merger and understand its need.More importantly, as John Rau suggests, she would have done independent fact finding which would give her an idea about who to talk to. Another important task she missed out was talking to Mr. Carpenter and exploring his views about the merger. If she had any knowledge about mergers she would have replied to Mr. Kellogg ¶s remarks on mergers and explained to him that acquisitions have far higher success rates than  µmergers of equals ¶. All these point to her lack of expertise in mergers and inexperience with conducting with clients. Royce Kellogg acknowledges how he always relied on Mort Meyer to deal with people problems.Further, his naive view of the merger, which is so far only an agreement between two heads, reinforces his skewed understanding of the problem. In response to numerous calls from employees, who had already been given heads-up, Kellogg was quick in drawing conclusion that consultants are stirring up trouble rather than understand the underlying causes. Mr. Kellogg ¶s belief that derivatives are harder than the assignment Susan is currently dealing with shows how little interest he has in organizational issues and understanding their importance.With only Mort ¶s death triggering the merger, it was pretty thin to start with. For two firms with almost equal share operating on similar lines of business, the only value that could be derived out of merger is to cut the costs through staff reductions and higher scale of operations. To realize this, one of the firms needed to be an underdog and everybody, including the consultants missed this entirely. Kellogg spoke about mergers of equals without realizing how dangerous it was. This evidence strongly suggests the desperate need for mergers and acquisitions expert.Hence, any corrective measure should start with bringing in an expert on mergers into the team. Statler should start with a fresh slate by bringing in a new team to work on the project and let go of the costs for the initial two weeks. This will also greatly alleviate Kellogg ¶s anger and frustration with the consultants. The new policies that should govern the Kellogg Champion should be centered on cutting costs and achieving a merit-based organization structure rather than keeping a set of policies and disregarding the other.
